The prognosis for a successful penetrating keratoplasty (PK) in eyes without inflammation prior to surgery approaches 80%. Nevertheless PK will not change the rate of HSV disease recurrence since it results from reactivation of latent virus lodged in the nerve ganglion.
Despite prophylactic oral antiviral therapy following PK for 12 months this patient suffered HSV keratitis relapse.
